For make a extreme angle photo you have to take it from an extreme position with you camera you have to focus the image from the top or the bottom or any place that you can but not in front of the person because that is going to look like a regular picture and is not going to be an extreme angle photo.From my extreme angle photo I liked how my model is standing next to the school logo and I thought that is a good focus not to extreme but is good.
